Medical Detox
Our Whittier detox unit was designed to feel nothing like a hospital ward. Set back from the street behind a courtyard garden, the space is deliberately quiet — soft lighting, private rooms, and a nursing station close enough for continuous monitoring without the institutional atmosphere that so often adds stress to an already difficult process. Over three to seven days, our medical team manages withdrawal with protocols built around the specific substance, your medical background, and any co-occurring mental health needs identified during your intake assessment.
Our medical team uses protocols developed from the original university research that founded TMC, updated quarterly against current outcomes data. Family members receive scheduled clinical updates from our charge nurse — never a black box.
What's included: 24/7 nursing, physician-managed medication, psychiatric consult within 48 hours, MAT when appropriate, and bilingual nursing staff.
Residential Treatment
A 30 to 90 day stay on our 40-bed Greenleaf Avenue campus. Our urban-sanctuary design pairs the convenience of the city with outdoor sanctuary space — nature paths, swimming pool, sauna, ceramics workshop, and a courtyard that doubles as our morning meeting space.
Days begin at 6:30 AM with a sunrise nature walk and structured outdoor fitness, followed by individual therapy, group work, and creative practice. The early-riser-outdoor rhythm is part of the clinical model — circadian regulation is one of the things our research showed strongly predicts long-term outcomes.
What's included: private and semi-private rooms, chef-prepared meals, equine-assisted therapy, art therapy, neurofeedback, family programming, and discharge planning that includes vocational rehabilitation work.
Outpatient Program
Three outpatient tracks for residents of Whittier, La Habra, Pico Rivera, Norwalk, and the wider Greater LA / North Orange County corridor.
PHP runs 5 days per week, 6 hours per day, for 2–4 weeks. IOP runs 3 evenings per week for 8–12 weeks — designed specifically for shift workers, first responders coming off duty, and parents who need to be home for school pickup. Standard outpatient continues weekly therapy and psychiatric medication management for 3–6 months after step-down.
Step-down between levels is determined by clinical milestones and outcomes data, not a fixed calendar.
Dual Diagnosis
Roughly two thirds of patients arriving at TMC have a co-occurring psychiatric condition driving or amplifying their substance use — PTSD, depression, anxiety, ADHD, bipolar disorder. For our veterans and first responders cohort, the rate is higher; trauma is the primary driver.
Our dual diagnosis track keeps psychiatric medication management and substance use therapy under one coordinated team. Dr. Tate reviews every plan, and the psychiatrist who prescribes is the same psychiatrist who attends the family meeting.
Approach: integrated assessment in week one, single coordinated treatment plan, Trauma-Focused CBT and EMDR for PTSD-driven cases, neurofeedback as adjunctive therapy, and a discharge plan that includes ongoing psychiatric follow-up — not just a referral list.

- 6:30 AM — Wake; sunrise nature walk on the campus paths
- 7:30 AM — Outdoor fitness or pool time
- 8:30 AM — Chef-prepared breakfast
- 9:30 AM — Individual therapy or psychiatry appointment
- 11:00 AM — Process group (CBT, MI, or 12-step facilitation rotation)
- 12:30 PM — Lunch and quiet courtyard time
- 2:00 PM — Specialized track (veterans, first responders, or family rebuilding)
- 3:30 PM — Creative practice (art therapy, ceramics, or equine work)
- 5:00 PM — Sauna or quiet reading hour
- 6:00 PM — Dinner
- 7:30 PM — Evening community meeting
- 9:30 PM — Wind-down; lights out at 10:00 PM
